Opt in to receive important messages from Neurotech regarding appointments and general information about our services.

Receiving text messages is optional. Neurotech will not condition starting or continuing any diagnostic procedure on your decision to receive or not receive text message. Choosing to not receive text messages will not affect payment, enrollment, or eligibility benefits from any source. If you opt-in to receiving text messages, you may opt out of receiving text messages from Neurotech at any time by following the instructions below under “STOP Information.” Consent cannot be revoked to the extent that action has already been taken in reliance on the consent.

RISKS Information

The text messages are not encrypted. Unencrypted text messages are not secure and could result in disclosure of protected health Information (“PHI”) to third parties. These text messages can be read by anyone with access to your phone or anyone who intercepts the text messages.

By opting-in to this service, you understand the above information, accept the risks associated with text messages, and consent to receive text messages.
